1. Acknowledgement of Risk
I understand that participation in basketball and related activities involves physical exertion and carries inherent risks, including but not limited to injury, illness, and in rare cases, death. I freely and knowingly assume all such risks on behalf of the participant.
2. Release of Liability
To the fullest extent permitted by law, I hereby waive, release, and discharge OTB Academies, its coaches, staff, volunteers, directors, agents, sponsors, and affiliated facilities from any and all claims, demands, liabilities, or causes of action that may arise from injury, loss, or damage incurred during or as a result of participation in the Program.
3. Medical Authorization
In the event of an injury or emergency, I authorize OTB Academies staff or representatives to seek medical treatment for the participant. I understand that I am responsible for any costs arising from such treatment.
4. Media Release
I grant OTB Academies permission to use photos, videos, or recordings of the participant for promotional, educational, or commercial purposes, in print or digital formats, without compensation. I understand these materials may be used on social media, websites, and marketing materials.
5. Code of Conduct
I agree that the participant will adhere to OTB Academies' code of conduct and respect coaches, teammates, opponents, referees, and facilities. Disruptive or unsafe behaviour may result in suspension or removal from the Program without refund.
6. Refund & Cancellation
I understand that registration fees are non-refundable and due in full unless otherwise stated in writing by OTB Academies.
